The one I saw was a little drawstring bag that slipped over everything and tied tight around the base like a cockring would do.

Yes, it covers cock and balls. Pubes show if you have any. I made some to wear at non nude beach. Hair tie sewn into cloth and goes around cock and balls like a cockring. Had MANY questions and comments. See it in my pics.

I think it depends on the jurisdiction. Many require the anal opening to be covered by at least one inch of fabric which you might be able to get by with some thongs.
