In hospital
I've recently finished a very long stay in hospital, being naked in bed for much of the stay. When the (female) Doc came to examine me I told her I was wearing nothing under the bed sheet. Not a problem, though later they asked me to wear a hospital gown (the sort that opens down the back).

Last year I had a 4 day stay in the hospital the best thing I got to wear a hospital gown and thats all
The only time I was in a hospital over night, my roommate removed his hospital gown as soon as he was under the sheet for the night and slept naked.
