Recent Skin Check visit
I've been using a dermatologist at an office of a large multi-state group practice since my former dermatologist sold his practice to this group. At these visits I notify the nurse that I don't need a gown. One time her response was that my underwear was OK to which I replied that would be coming off as well. The doc has no problems with me being fully nude for the exam.
On this last visit I told the nurse (not the same one) that I didn't need the gown and that I would be removing everything. This time the response was "Perfect" which surprised me and I interpreted as meaning she (the nurse) agreed with my philosophy regarding these exams. When the knock came at the door the doc stuck her head in the door and asked me if a med student could accompany her. My answer was "Of course" and in came the student and the nurse as scribe/chaperone. I realized that her comment of "Perfect" probably had been that I would be a perfect example patient for this future dermatologist.
I am very pleased with this particular dermatologist since she evidently approves of my choice of non-apparel for these exams. From my perspective, this is the right choice since hiding any part of my body is harming only me.

My dermatologist's assistant always told me to strip to my underwear so that id what I did.
When I stopped wearing underwear I wore a swim brief.
I did tell the doctor that I was often nude outdoors so he would quickly pull the underwear aside and check.
Eventually I asked him if he would be OK examining a nude patient.
He said that it actually results in a better exam but he doesn't have his staff asked his patients to get naked because he knows that it would make too many of them uncomfortable. I haven't worn anything for my exams since. Should have asked sooner.
I've been seeing my dermatologist twice a year for about 8 years. I told him at my first appointment that I live as a nudist and often get overall sun. I also said I don't need a gown (they don't even ask anymore). He did say that he preferred I leave my underwear on until he enters the room as the the doors to the exam rooms open to an open hallway where patients are walking by. Once he and his assistant enter the room, I remove my underwear. He does examine my penis and I have had a couple spots he photographed for reference. He once did a biopsy on one spot (it turned out to be non-cancerous). When he was giving me a shot to deaden the area he said, you'll feel a quick pinch. I said, I have a tattoo on my penis so a shot will be nbd. He laughed and said, I could never get a tattoo there. lol
