When my dad was a kid in the deep country before mortuary services were universal, it was the practice that a group of men would gather - he was one of them, once, when the local grocer died - to wash and dress the bodies of male deceased, and women for the women - the gender precaution due to the nudity involved.

Pacemakers have to be removed, which the mortuary will do. Doubtless not the neatest of surgeries...

No idea. The mortuary did not discuss with us what happened to my dad's pacemaker after they took it out.

Well not sure if I'll die naked, but already warned my wife that I want to be buried naked. They may cover me with flowers inside the coffin but not even underwear please!

Morbid and turgid talk ahead - I seem to recall someone in the business I used to hang with mentioning that it is far from uncommon for a male's genitals to become and remain partially or fully erect postmortem, making an open casket nudist funeral rather, let's say, a tad more revealing than appropriate.
Many open casket funerals are only open from the waist up.
