KNICKERS FOR SEXUAL STIMULATION
In the nineteenth century, knickers had no gusset. They were in two attached halves with an open crotch, hence 'pair of knickers', 'pair of panties' etc. The open crotch was to allow going to the toilet without removing them or any of the heavy clothing fashionable at the time. There is some imprecision in defining what is incontinence. How much leakage makes someone incontinent? How often must a considerable accident have to occur for someone to be incontinent? Since puberty at 13 I used paper towels and other extra protection for both outlets.

Types of Incontinence Wear: PULL-UPS
There are many kinds of adapted pants and knickers on the market for light incontinence. There are many brands of washable or disposable pants and knickers adapted to take pads or with absorbent areas. Some are intended to be single-sex. The pads when sold separately can be single- or unisex. Fixation pants and knickers are non-absorbent; it is the incontinence pad that they 'fix' (hold in place) that is absorbent. Many are unisex - no front exit. The manufacturers often just refer to them by their brand name chosen to give a hint of what they are.
They are more stretchy than is usual for ordinary underwear and so take a pad more readily. The pad can be unisex or female according to the length at the front. Ladies who have heavy incontinence might prefer a longer front as for unisex pads.
Most kinds of pants or knickers for light incontinence are pull-ups but some can be used for much heavier leakages.
Whether the pad is fixed to the pants, such as in a pocket, or just covered by them might be significant for those who have an active lifestyle. Some fixation pants allow the pad to be moved a little in them.

Types of Incontinence Wear: DIAPERS
These open out flat. A large absorbent pad to covers the area likely to be affected by urine or faeces. Usually they have plastic sides with one or two adhesive tabs, or in some kinds of diaper, 3 or 4 studs on each side to hold them securely around the body between the groin and the waist. Diapers can be bulky: some users would feel that they would not like to wear close-fitting trousers or other outer wear which would reveal the outline. They can also make a crinkling sound as the wearer moves.
Cloth diapers (USA)/nappies (UK) are made of square, absorbent, washable material held in place by safety pins or something more modern. There are many brands of cloth diapers/nappies shaped like plastic ones with fastening studs at each side. Occasionally they are referred to as 'napkins' but in the UK, this is more usual for smaller squares of lighter material used at the dinner table to clear up stray bits of food from around the mouth or fingers or to prevent the soiling of clothes - similar in purpose to nappies but at the other end of the alimentary canal. In the UK, 'nappies' usually refer to babies: some adults would be offended if their incontinence wear were referred to in this way.
Some kinds of incontinence wear consist of a waistband to which a pad with adhesive patches fixes to it. I am unsure whether these are pull-ups or diapers or neither; they are just pads with a means of holding them in place but not covering them. I was issued with some on an overnight hospital stay: on returning home I got rid of them as soon as possible.
Pants made of plastic, rubber or other waterproof material can be worn over any of these types of incontinence wear for extra protection.
Diapers, pull-ups, fixation pants, nets or knickers with many brand names are really all variations of ways to hold a pad of absorbent material in place and to be able to get access to it as needed. The differences between the variety of incontinence wear amount to a few features: amount of absorbency; whether they open with tapes or studs (diapers), usually at the sides or whether they are sufficiently stretchy so that no opening is needed (pull-ups or fixation pants); material used: waterproof (usually plastic or rubber) or textile; washable or disposable. This is likely to affect cost. My washable fixation pants cost very little - around 1 each depending on brand, type and quantity. I use only about six per year. I use one disposable pad per day costing around 35p.

A PERSONAL CHOICE

Although I do not have particularly big genitals, I could not find any thongs which covered me pointing upwards; so I got to work on an old sewing machine and made some. UNISEX KNICKERS
I have often wondered why so much underwear is marketed as male or female. Why not just leave it to the customer to decide what they would like to wear? Some advertised as male have no front exit or gusset: there seems to be nothing specifically male about them. Often I have thought that I would like to wear something like women's plain white maxi-briefs. Ladies' knickers are better for peeing over the top or sitting down because of their more stretchy, more durable waistband. The gusset, however, would need to be either in two forms, male and female: to be genuinely unisex, there would need to be a detachable gusset or pad. Pads have the advantages of being able to adjust the amount and location of protection and the knickers do not need to be changed so frequently. For these reasons, unisex knickers with pads are my preferred choice.
In 2013 I had an accident resulting in spinal injury at the base of my neck with paralysis from there down, spasticity (muscle contractions and spasms) and double incontinence. Both my anus and my urethra have been left closed except with manual intervention: I hardly ever pass urine via the usual route (I have a suprapubic catheter); nurses visit on alternate days to give me a bowel evacuation.
My current choice is white unisex knickers (Abena Abri Fix large ref: 9251 or Allanda iD Expert Ultra Large ref: 5400300250). They can be washed many times and last more than three months. They are made of lightweight lycra netting, and having no gusset, are intended to hold a pad - effectively a disposable gusset. I wear an Abri San Premium 5 pad, big enough to extend from an inch or so below my navel to the top of my bum crevice, capacity 1,200 ml. (Other pads with larger capacities are available). These open out flat. A large absorbent pad to covers the area likely to be affected by urine or faeces. Ladies' knickers are better for peeing over the top or sitting down because of their more stretchy, more durable waistband. To be genuinely unisex, there would need to be a detachable gusset or pad. Pads have the advantages of being able to adjust the amount and location of protection and the knickers do not need to be changed so frequently. In 2013 I had an accident resulting in spinal injury at the base of my neck with paralysis from there down, spasticity (muscle contractions and spasms) and double incontinence. With my foreskin permanently back, I can rub all my erogenous parts against my pad to give a sense of well-being. My pad soaks up semen overflows and occasionally, ejaculations, but only very rarely do I pass any urine into it. I have no more leakages than I had before my accident but I am now less able to deal with them.
I find that knickers such as these tend to be tight around my groin, the boundary between the top of my leg and my pubic triangle and cheeks, so I buy a size larger than my waist measurement indicates (I am about 37-33-39). They can be pulled up to my waist, when the sides are about 8 inches or rolled down to 2 or 3 inches.
Since I was given some by a lady friend, I have also been wearing Abri Fix pants super Large ref: 90693. These have very short legs which I roll up into my groin and along the base of my cheeks to form briefs. They reduce the issue of tightness. Like the nets, they are made of polyester and elastane but the thicker material makes the pad hardly visible. There is some imprecision in defining what is incontinence. How much leakage makes someone incontinent? How often must a considerable accident have to occur for someone to be incontinent? Since puberty at 13 I used paper towels and other extra protection for both outlets.

Types of Incontinence Wear: DIAPERS
These open out flat. A large absorbent pad to covers the area likely to be affected by urine or faeces. Usually they have plastic sides with one or two adhesive tabs, or in some kinds of diaper, 3 or 4 studs on each side to hold them securely around the body between the groin and the waist. The thoughts going through my mind have always been protection of my outer clothes, arousal and appearance. Finding the underwear of my choice has been a long journey; perhaps it is not finished. I started with Marks and Spencer's male white briefs. I liked their appearance; the front gusset extending up to the waist allowed my penis to point upwards; the hollow knitted waistband was durable and allowed me to pee over the top or to take them down easily. I have also tried other kinds and brands with a front exit in the form of an inverted Y. I have never really liked front exits: they fit only a limp penis pointing downwards and even then I did not find that I could urinate conveniently through the exit. I felt that they left my male parts scrunched up in a small bag rather than something to hold up and enjoy.
Then I tried thongs. As I tend to put on weight around my hips and thighs, I thought they might suit me. Although I do not have particularly big male parts, I could not find any thongs which covered me pointing upwards; so I got to work on an old sewing machine and made some. I like the feel in my nick and bare cheeks, although sometimes I feel a little soreness when worn all day. Maybe I should not have shaved.
UNISEX KNICKERS
I have often wondered why so much underwear is marketed as male or female. Why not just leave it to the customer to decide what they would like to wear? Some advertised as male have no front exit or gusset: there seems to be nothing specifically male about them. Often I have thought that I would like to wear something like women's plain white maxi-briefs. Ladies' knickers are better for peeing over the top or sitting down because of their more stretchy, more durable waistband. To be genuinely unisex, there would need to be a detachable gusset or pad. Pads have the advantages of being able to adjust the amount and location of protection and the knickers do not need to be changed so frequently. For these reasons, unisex knickers with pads are my preferred choice.
In 2013 I had an accident resulting in spinal injury at the base of my neck with paralysis from there down, spasticity (muscle contractions and spasms) and double incontinence. Both my anus and my urethra have been left closed except with manual intervention: I hardly ever pass urine via the usual route (I have a suprapubic catheter); nurses visit on alternate days to give me a bowel evacuation.
My current choice is white unisex knickers (Abena Abri Fix large ref: 9251 or Allanda iD Expert Ultra Large ref: 5400300250). They are quite hard-wearing and can be washed many times. They are made of lightweight lycra netting, and having no fixed gusset, are intended to hold a pad - effectively a disposable gusset. I wear an Abri San Premium 5 pad, big enough to extend from an inch or so below my navel to the top of my bum crevice, capacity 1,200 ml. (Other pads with larger capacities are available). With my foreskin permanently back, I can rub all my erogenous parts against my pad to give a sense of well-being. My pad soaks up semen overflows and occasionally, ejaculations, but only very rarely do I pass any urine into it. It is also an insurance against occasional bowel leakage when I have eaten something that does not agree with me. I have no more leakages than I had before my accident but I am now less able to deal with them.
I find that knickers such as these tend to be tight around my groin, the boundary between the top of my leg and my pubic triangle and cheeks, so I buy a size larger than my waist measurement indicates (I am about 37-33-39). They can be pulled up to my waist, when the sides are about 8 inches or rolled down to 2 or 3 inches.
Since I was given some by a lady friend, I have also been wearing Abri Fix pants super Large ref: 90693. These have very short legs which I roll up into my groin and along the base of my cheeks to form briefs. They reduce the issue of tightness. Like the nets, they are made of polyester and elastane but the thicker material makes the pad hardly visible. Pants with very short legs which can be worn as briefs are sometimes called cuff knickers or boy shorts.
